The information below will help you prepare for your first appointment and installation of your SCRAM device.

Step 1

Call for an appointment – (815) 436-3636.

Step 2

Fill out our Client Intake Form completely.

Step 3

Bring drivers license, current pay stub, W-2, and court order.

Step 4

One-time installation fee of $100, plus first 30 days of monitoring fees is due at time of installation. We accept cash, check, and credit cards.

Future Appointments

At all future appoints the following may take place:

• Collect any monies due
• Discuss any necessary schedule changes
• Review activity reports
